How Do I Make an Appointment?

To schedule an appointment, please use our contact form on our website or contact our office directly and speak with a staff member. We will work with you to schedule an appointment that fits your schedule.

How Long Do I Have to Wait for My Appointment?

We strive to see all patients within 24 hrs. We will do our best to see patients in need on the same day.

Does Insurance Pay for My Treatment?

Insurance may cover your pain treatment at ARILEX Center for Endometriosis & Pelvic Pain. However, it is important to note that your visit may not be covered by your insurance plan without a referral from a physician.

Can I Fill Out My Patient Forms Ahead of Time?

Yes. We encourage you to download your patient forms from our website and fill them out prior to your scheduled appointment. If they are not completed in advance, please arrive 30 minutes prior to your appointment time to complete this information. Failure to do so may result in the delay of your appointment time.

What Should I Bring to My Appointment?

New Patients should arrive no less than 30 minutes prior to their appointment and always bring their driver’s license and proof of insurance.

Will I Receive Pain Medication at My First Visit?

There is no guarantee that patients will receive pain medication on their first visit. Dr. Gary G. Theofilis will perform a thorough examination before prescribing any form of treatment.
